WHAT DO YOU THINK THIS IS? 14yo male, past medica $WHAT DO YOU THINK THIS IS? 14yo male, past medical history of asthma but otherwise well presented to ED following a soccer injury where he collided with another player and sustained a 5cm scalp laceration to the right parietal region. Vaccinations were up to date including tetanus and there were no signs of concussion. The laceration was sutured and he was discharged home. 5 days post, he returned to ED febrile with temp of 39 degrees, drowsy, with pain over the suture line and copious purulent discharge. The wound was cleaned and the patient given oral Augmentin and discharged home with instructions of when to return to ED. 3 days later the patient returns to ED. Patient is afebrile following oral Abs however complains of ongoing local pain and purulent discharge from the suture site. WHAT WOULD YOU DO NEXT? FBE showed elevated WCC and neutrophils and CRP was elevated. The patient was admitted for IV Abs and monitoring. 24 hours later the clinical picture was similar with no improvement. A head CT was performed to assess the potential of an underlying skull fracture or spread of infection causing osteomyelitis. The following images are the findings. WHAT DO YOU THINK THIS IS? That’s right. It’s a tooth. The tooth was removed. Patient recovered fully and was discharged 48 hours later completing a total of 7 days of IV Abs. https://casereports.bmj.com/content/12/9/e230525$
